NOTICE OF PRIVACY PRACTICES Practice: Glazer Facial Plastic & Cosmetic Surgery Effective Date: February 2, 2026
This practice is a HIPAA-covered entity. Most patient information is protected health information (PHI) and is governed by HIPAA privacy rules. PHI is generally exempt from the Indiana Consumer Data Protection Act (INCDPA).
1. Indiana Consumer Data Protection Act (INCDPA) – Limited Non-PHI Data
For any non-medical personal data (such as website visits, marketing emails, or contact forms), Indiana residents have these rights:
- Confirm whether we are processing your personal data and access that data
- Correct inaccuracies in your personal data
- Delete personal data you provided
- Obtain a portable copy of your data in a readily usable format
- Opt out of processing for targeted advertising or profiling
We do not sell personal data. We will respond to any request within 45 days (we may extend by an additional 45 days if needed and will notify you).
2. Biometric Information
We collect high-resolution facial photographs and 3D scans solely for treatment planning, mapping, and progress tracking. These images are considered PHI under HIPAA. They are not classified as “Biometric Data” under INCDPA because they are collected for healthcare treatment purposes and/or derived from photographs (both excluded under INCDPA).
3. 42 CFR Part 2 – Substance Use Disorder Records
If any of your records relate to substance use disorder (SUD) treatment received at a federally assisted Part 2 program, they are subject to strict federal confidentiality protections. These records (or testimony about them) generally cannot be used or disclosed in civil, criminal, or administrative proceedings against you without your specific written consent or a court order.
4. Treatment Risks & Informed Consent
A separate Informed Consent form must be reviewed and signed before any treatment. Key risks include:
- Botox / Dysport: bruising, headache, eyelid drooping, rare toxin spread (difficulty breathing/swallowing)
- Dermal Fillers: swelling, lumps, vascular occlusion (can cause skin necrosis or blindness)
- Sculptra / Radiesse: delayed nodules, injection site tenderness
